Document list (first cycle)

  1. Application for acceptance to I degree studies generated in the IRK system
  2. Document confirming candidate’s identity (passport)
  3. Visa or other document authorizing legal residence on the territory of Poland
  4. A certificate of secondary education, with grades list attached
  5. Apostille or legalization of the certificate together with grades list
  6. Sworn translation of secondary education certificate into Polish language, or a translation certified by a Polish consul, together with grades list
  7. Information that the foreign certificate entitles the candidate to start post-secondary studies in the issuing country
  8. Sworn translation into Polish language, or a translation certified by a Polish consul, stating that the candidate’s certificate authorizes him/her to apply to University in the issuing country
  9. Authorization of the certificate by an Education Supervision Body. Does not apply to documents issued in EU, OECD, EFTA, EOG countries, or such countries with which Poland has agreements on education authorization
  10. Document confirming knowledge of English language
  11. Insurance policy against sickness and accidents for the period of candidate’s education in Poland, or European Health Insurance Card, or agreement with National Health Fund
  12. 3 up-to-date photos of the candidate (size: 35x45 mm; on the back of each of the photos there should be written the candidate’s name and additionally on one of them – permanent residence address
  13. Confirmation of payment of recruitment fee

Document list (second cycle)

  1. Application for acceptance to II degree studies generated in the IRK system
  2. Document confirming candidate’s identity (passport)
  3. Visa or other document authorizing legal residence on the territory of Poland
  4. A first cycle studies diploma, with grades attached
  5. Apostille or legalization of the diploma together with grades list
  6. Sworn translation of first cycle studies diploma into Polish language, or a translation certified by a Polish consul, together with grades list
  7. Information that the foreign diploma entitles the candidate to start post-secondary studies in the issuing country
  8. Sworn translation into Polish language, or a translation certified by a Polish consul, stating that the candidate’s diploma authorizes him/her to apply to University in the issuing country
  9. Recognition of diploma. Does not apply to documents issued in EU, OECD, EFTA, EOG countries, or such countries with which Poland has agreements on education authorization
  10. Document confirming knowledge of English language
  11. Insurance policy against sickness and accidents for the period of candidate’s education in Poland, or European Health Insurance Card, or agreement with National Health Fund

Document list (third cycle)

  1. Application for acceptance to PhD degree studies generated in the IRK system
  2. Document confirming candidate’s identity (passport)
  3. Visa or other document authorizing legal residence on the territory of Poland
  4. Résumé or CV
  5. Copy of diploma of completing BA and MA studies, or uniform Master’s studies in the fields of technical, mathematical, physical, chemical, economic or social sciences in courses that are compatible or related to the scientific discipline for which the candidate is submitting an application for as PhD degree studies.
  6. A candidate for PhD degree studies that is not in possession of a diploma of completing MA degree studies submits a document attesting to the acquisition of the status of a beneficiary of the program entitled: “Diamond Grant”.
  7. Apostille or legalization of BA and MA  diplomas together with grades list
  8. Sworn translation of diplomas (BA and MA) into Polish language, or a translation certified by a Polish consul, together with grades list
  9. A document confirming the change of surname ( this relates to people having a diploma issued under another name than that provided in the application for PhD studies.
  10. Sworn translation into Polish language, or a translation certified by a Polish consul, stating that the candidate’s diplomas (BA and MA) authorizes candidate to apply to University in the issuing country
  11. Recognition of diplomas (BA and MA). Does not apply to documents issued in EU, OECD, EFTA, EOG countries, or such countries with which Poland has agreements on education authorization
  12. Photographs against a bright background with no headgear (45 mm x 65 mm).
  13. Insurance policy against sickness and accidents for the period of candidate’s education in Poland, or European Health Insurance Card, or agreement with National Health Fund
  14. A summary of professional accomplishments including information relating to the following: reasons for applying for acceptance for doctoral studies, envisaged subject matter of future PhD thesis, scientific and organizational achievements attained (if the candidate has some).
  15. Document confirming knowledge of English language
  16. Consent for the processing of personal data in accordance with the Act dated 29 August 1997 relating to the protection of personal data (Law Gazette no. 133, pos. 883 with further amendments) (print-out).
  17. Confirmation of payment for the first semester of studies
